This article presents a radically empirical and descriptive account of the most often neglected viewpoint: that of the 'doer'. Using field autoethnographic examples from remote Northern Brazil, the article portrays the clash between the universalistic programs of international agencies and doer perspective at the fieldwork level. The see-judge-act tool helped to chart the heuristic field of daily life as a reflective practice. Three moments illustrate the confrontation of the action planned: (a) a projection of abstract theoretical assumptions based on a universalistic framework; (b) a silent period of accommodation of local realities to the imposed, descending logic of the formal discourse; and (c) the production of an exportable version of the partially hidden local situation. All three moments show the struggles to make the 'doer viewpoint' communicable beyond the local geographic region. In the end, fieldwork dangers were censored and inconsistencies were evened-out.

Les agences canadiennes de santé publique recourent à la mobilisation d'acteurs communautaires pour mettre en oeuvre leurs programmes au plan local. La logique sociétale de programmation diffère toutefois de la pratique communautaire de production et de mise en oeuvre des connaissances. Der vorliegende Beitrag behandelt ein qualitatives Evaluationsprojekt, das sich mit partizipativen Bildungsprozessen von Prostituierten in einer entlegenen brasilianischen Region befasste. Im Zentrum des Projektes standen die Bedeutung unvorhergesehner Faktoren auf die Evaluationsergebnisse, die Bedeutsamkeit kollektiver Lernformen sowie die aktive Teilhabe lokaler sozialer Akteure bei der Gesundheitsprävention. Für die Evaluation wurden teilnehmende Beobachtung sowie individuelle und Gruppeninterviews in einer ethnographischen Perspektive genutzt unter Hinzuziehung gängiger lateinamerikanischer Konzepte von Volksbildung für die Rekonstruktion kultureller Sinngebung. Im Rahmen der Studie wurden zunächst unbeabsichtigte sekundäre Einflussfaktoren erkennbar, z.B. die Entwicklung von wechselseitigen Hilfepraktiken und Änderungen in individuellen und kollektiven Lebensverläufen, sowie Änderungen in den kollektiven Bedeutungszuschreibungen, die aus der (Wieder-) Gewinnung einer gemeinsamen kollektiven Geschichte erwuchsen. Dabei erlaubte die Nähe zu den lokalen Gegebenheiten die Teilhabe der Forschenden unmittelbar im sozialpolitischen und -kulturellen Programmkontext. Dies wiederum verbesserte die Möglichkeit, ungeplante Faktoren mittels eines qualitativen und partizipativen Ansatzes zu identifizieren. In order to reduce social inequities in health, nurses need to move from patient advocacy to policy advocacy. Literature reports many barriers when nurses try to address the structural level of policy advocacy. Very little empirical research exists to show how nurses respond to these barriers and the resulting impact on the effectiveness of policy advocacy. This study characterized the types and the process of advocacy practiced by community health nurses. A collaborative ethnography with 21 nurses showed that their policy advocacy was often dismissed, leading to negative consequences such as less policy advocacy attempts, changes in their clinical evaluation with patients and personal experiences of moral distress. We argue that structural change will only occur when collective advocacy will be supported by organizations, therefore reducing inequities that affect both community health and working conditions of nurses.
Afin de réduire les iniquités sociales de santé, les infirmières doivent dépasser le plaidoyer axé sur les individus en investissant des stratégies de plaidoyer ciblant les structures et les politiques. Dans cet article, nous soutenons que de ne pas exercer de plaidoyer structurel peut amener des conséquences néfastes pour les infirmières elles-mêmes et les communautés qu'elles desservent. Cette étude caractérise les types et le processus de plaidoyer exercés par les infirmières en santé communautaire. Une ethnographie collaborative avec 21 infirmières montre que leurs tentatives de plaidoyer structurel étaient ignorées, décourageant les tentatives futures, entraînant des changements dans leur évaluation clinique et des expériences de détresse morale. Nous argumentons que le changement structurel surviendra seulement lorsque les stratégies de plaidoyer seront collectivisées et soutenues par les organisations de santé, réduisant ainsi les iniquités affectant à la fois la santé communautaire et les conditions de travail des infirmières.

Jointly conducted with a coalition of HIV/AIDS community-based organizations (CBOs), this evaluability assessment sought to better understand the factors that affect the feasibility of a participatory program evaluation to be undertaken in partnership with the CBOs' non-governmental-organization members and public-health agencies. Participatory evaluations and partnerships are grounded in social and institutional authority structures that unavoidably influence researchers and evaluators. The construction of a theoretical framework for socio-political evaluability assessment of participatory evaluations is a necessary precondition for the coalition's members to engage effectively in evaluation research with other partners.
